AIM: This prospective cohort study aimed to investigate whether orthodontic appliance removal (OAR) combined with caries-preventive strategies and fluoride varnish treatments alters salivary physicochemical properties, changes the activity of carbonic anhydrase VI (CA VIACT ) and α-amylase (α-AMLACT ), and favors the regression of active caries lesions (ACL). DESIGN: Twenty-two individuals aged between 13 and 24 years were assessed for the presence of visible biofilm, daily sugar exposure, caries activity, salivary flow rate (SFR), pH, buffering capacity (BC), and CA VIACT and α-AMLACT activity at baseline, and 1, 5, and 13 weeks after OAR. Variables were assessed using repeated-measures analysis of variance, Cochran's Q and McNemar's test, and Pearson's correlation. RESULTS: We observed a significant decrease in the number of ACL at the 5-week (29% reduction) and 13-week follow-ups (58% reduction). At the 5- and 13-week follow-ups, the percentage of visible biofilm and sugar exposure decreased, whereas the salivary pH and α-AMLACT activity significantly increased. BC and CA VIACT remained unchanged throughout the follow-up. CONCLUSION: OAR combined with caries-preventive strategies and fluoride varnish treatments favored the regression of ACL and increased salivary pH and α-AMLACT activity, whereas BC and CA VIACT remained stable.

BACKGROUND: This research aimed to evaluate the salivary concentrations of fluoride (F-), calcium (Ca2+), and phosphate (Pi) after brackets bonding, and to identify the role of [F-], [Ca2+], and [Pi] on the development of active caries lesion (ACL) in individuals under fixed orthodontic treatment. MATERIAL AND METHODS: A longitudinal investigation with twenty-two individuals from 11 to 22 years of age was performed in four phases (baseline and after 1, 3, and 6 months). Analyses were carried out considering the salivary concentration of [F-], [Ca2+], and [Pi], as well as the caries index. Data were analyzed using the Friedman test, followed by the Wilcoxon test and the multivariate Cox model (p≤0.05). RESULTS: 1 and 3 months after appliance bonding, the [Ca2+] was statistically lower than after 6 months (p<0.0083). On the other hand, salivary [F-] and [Pi] did not show any significant difference during the follow-up. The Cox model demonstrated that the increase of 1 µg/mL in Ca2+ decreased the risk of ACL development by 27%. In conclusion, the levels of Ca2+ changed during orthodontic treatment. CONCLUSIONS: A high Ca2+ level in the saliva is a protective factor for ACL development over time. Key words:Adolescents, bioinorganic chemistry, dental caries, orthodontic appliances.

Objective: This study aimed to investigate if ß-cyclodextrin nanoparticles potentiate the photodynamic antimicrobial chemotherapy (PACT) effects in single and microcosm oral biofilms using methylene blue (MB) and a red laser. Background data: Studies of PACT have demonstrated promising effects; however, the association of nanoparticles with photosensitizers could enhance the antimicrobial result. Materials and methods: Biofilms were grown on enamel blocks either with Streptococcus mutans or in a microcosm model (salivary microorganisms) supplemented with sucrose. PACT using 50 µM MB associated or not with 32 µM encapsulated ß-cyclodextrin with MB for 5 min, followed by irradiation with red laser (λ = 660 nm, 320 J/cm2), was conducted and the counts of viable microorganisms in proper selective media were determined. Data were analyzed by one-factor ANOVA, followed by Tukey's test, or Kruskal-Wallis test, followed by Dunn's post hoc test, all with a significance level of 5%. Results: In the single-species biofilm model, a significant reduction in S. mutans counts was found for all groups when light was present. In the microcosm biofilm model, no significant difference was found among the groups for total streptococci, but a significant reduction of S. mutans was observed for the PACT group of encapsulated ß-cyclodextrin+MB. However, no statistically significant difference was observed among the PACT groups. Conclusions: PACT with ß-cyclodextrin mediated with MB associated with a red laser reduced S. mutans in microcosm biofilms. However, the presence of ß-cyclodextrin nanoparticles did not potentiate the PACT effects in single or microcosm oral biofilms.

RESUMEN

Objetivo: Relatar um caso clínico envolvendo a abordagem cirúrgica/restauradora e medidas preventivas. Relato do caso: Paciente MEMO, 6 anos e 8 meses de idade, feminino, com alto risco/atividade de cárie, apresentando biofilme visível em superfícies lisas livres, lesões de cárie ativa, perda precoce do dente 85, fístula relacionada com a região do dente 75, mudança de cor do dente 61 e perda significativa de dimensão vertical posterior, apresentou-se para tratamento na Clínica de pós-graduação em Odontopediatria da FOP/Unicamp. O plano de tratamento consistiu na abordagem integral focada na promoção da saúde como desfecho principal, ou seja, mudança de comportamento em relação à saúde bucal, e como desfecho secundário, a abordagem cirúrgica/restauradora. Instrução de higiene bucal foi realizada em todas as sessões. O manejo do comportamento foi conduzido por meio da evidenciação e controle de biofilme, profilaxia e adequação do meio bucal seguido da extração do dente 75, selamento de fossas e fissuras dos dentes 36, 46 e 16 e restaurações no 55 e 65. Mantenedor de espaço removível inferior foi realizado. Conclusões: Após seis meses pôde-se observar ausência de biofilme visível, o índice de placa estava controlado e não havia evidência de novas lesões de cárie. Pode-se concluir que o sucesso obtido neste caso destaca a importância da abordagem integral focada na promoção da saúde na clínica odontopediátrica.


Objective: To report a clinic case involving the surgical/restorative approach and preventive actions. Case report: The patient MEMO, aged 6y and 8mo, female, with high caries risk/activity, presenting visible biofilm on free smooth surfaces, active carious lesions, early loss of the 85 tooth, fistula related with the 75 tooth region, color change on 61 tooth and significant loss of posterior vertical dimension has been driven to Pediatric Dentistry Graduate Clinic at FOP/ Unicamp. Treatment plan consisted on integral approach focused as primary outcome on health promotion, behavior change in relation to oral health, and as secondary outcome, the surgical/ restorative approach. Oral hygiene instruction was conducted in all sessions. The behavior management was conducted by evidence and control of visible dental plaque, prophylaxis and oral environment adequation; extraction of 75 tooth, pit and fissure sealing of the 36, 46, 16 teeth, and restorations in the 55 and 65. Removable lower space maintainer appliance was made. Conclusions: After 6 months it could be observed that there was not visible biofilm, the IP was controlled. In addition, there was no evidence of new caries lesions. It could be concluded that the success obtained on this clinical case bring about the importance of the integral performance focused on healthy promotion in pediatric dentist clinic.

O presente trabalho tem a finalidade de relatar um caso orto-cirúrgico de uma paciente com má oclusão do tipo Classe III associada a uma mordida aberta anterior. O plano de tratamento foi estabelecido em quatro fases, a pré-cirúrgica, a transcirúrgica, a pós-cirúrgica e a fase de contenção. Após o diagnóstico, todas as metas do tratamento foram estabelecidas e discutidas de maneira conjunta entre o ortodontista e o cirurgião bucomaxilofacial. Na fase pré-cirúrgica foram feitos todos os procedimentos para a descompensação dentária em ambos os maxilares. O procedimento cirúrgico consistiu na rotação anti-horária da mandíbula por meio da osteotomia sagital do ramo mandibular juntamente com mentoplastia. Na fase transoperatória foi conseguida a intercuspidação dentária com obtenção de um trespasse vertical e horizontal adequados pelo ato cirúrgico e por procedimentos fisioterápicos até que fosse retomada a função ideal do sistema estomatognático. Após esta fase, a paciente retornou à clínica ortodôntica para que fosse submetida aos procedimentos de finalização. Foi colocada uma contenção superior e inferior.


The present study aims to report an ortho-surgical case of a patient with Class III malocclusion associated with an anterior open bite. The treatment plan was established in four phases, the pre-surgical, trans-surgical, post-surgical and the retention phase. After the diagnosis, all treatment goals were established and discussed jointly between the orthodontist and maxillofacial surgeon. At pre-surgical phase all procedures for dental decompensation were performed in both jaws. The surgical procedure consisted on a counterclockwise rotation of the mandible by sagittal osteotomy of the mandibular ramus with mentoplasty. At transoperative dental intercuspation was achieved through vertical and horizontal overlap suitable for surgery and physiotherapy procedures until this resume optimal function of the stomatognathic system. After this phase, the patient returned to the clinic to complete the procedure. An upper and lower retainer was placed.

O presente artigo tem a finalidade de relatar um caso clínico de uma paciente afro-brasileira, dolicofacial, 18 anos de idade, com mordida aberta anterior (MAA) dentoalveolar associada ao hábito de sucção digital. O tratamento ortodôntico consistiu na colocação de um aparelho ortodôntico fixo com bráquetes Edgewise e controle vertical com o uso do Gancho J com puxada alta somente na arcada inferior, com a finalidade de rotação anti-horária da mandíbula e o consequente fechamento da mordida. Simultaneamente, a paciente foi encaminhada para tratamento fonoaudiológico. O tratamento foi finalizado com êxito ao final de 36 meses com a correção da MAA havendo melhora das relações vertical e sagital entre a maxila e a mandíbula, obtenção de estética, equilíbrio e harmonia facial, além das características oclusais e funcionais. Assim, foi possível concluir que após a remoção do hábito de sucção digital é necessário que seja feita uma intervenção multidisciplinar que, neste caso, consistiu numa intervenção ortodôntica associada a um acompanhamento fonoaudiológico. Além disto, o uso de um extrabucal, colocado diretamente no aparelho ortodôntico na arcada inferior (Gancho J) foi primordial no fechamento da mordida, pois houve um controle vertical, o que proporcionou uma rotação mandibular no sentido anti-horário e o consequente fechamento da mordida.


This article aims to report a case of an Afro-Brazilian patient, dolicofacial, 18 years of age, with open bite dentoalveolar associated with digit sucking habit. Orthodontic treatment consisted on a fixed orthodontic appliance with edgewise brackets and vertical control using the J hook with high pull only in the lower jaw for the purpose of anti-clockwise rotation of the mandible and the consequent closing of the bite. Simultaneously, the patient was referred for speech therapy. The treatment was completed successfully at the end of 36 months with the correction of open bite with an improvement of vertical and sagittal relations between the maxilla and the mandible obtaining facial aesthetics and harmony besides the occlusal and functional characteristics. Thus, it was concluded that after breaking digit habit, a multidisciplinary intervention is required.In this case it consisted on an orthodontic intervention associated with a speech therapist. In addition, the use of a headgear placed directly on the archwire, the lower arch in the case was the J hook, was paramount in closing the bite because there was a vertical control which provided a mandibular rotation in a counterclockwise direction and the consequent closing of the bite
